Passport Renewal

Who qualifies?
You may obtain a new United States passport using application form DSP-82, provided you meet all five of the following requirements:

• You are at least 14 years old at the time of application;
• Your most recent passport was issued within the last 15 years;
• Your most recent passport was not limited, i.e. it was issued for its full five or 10 year validity;
• You are able to submit the passport with this application; and
• You are not requesting a change in the passport data, other than a name change due to marriage or divorce.


What documents are required?

• Your most recently issued passport.
• Evidence of change of name: If you have changed your name since the last passport was issued please provide your original marriage certificate, divorce decree or court order which restores a previous or maiden name. This document will be returned with the new passport.
• A completed passport form DSP-82. For children under the age of 14, form DSP-11 is required.
